Forms

Change of Status.
Fill out a Change of Status form if you need to update your mailing address or name or your proctor's name or address. It is essential that we have the most current information for both you and your proctor on file.

Transcript Request.
Students needing official transcripts must request one in writing. Submit the Transcript Request form to have an official transcript generated for you.

Local School Approval.
If students are planning on transferring credit to their local school or learning organization, they will need to seek prior approval from the school. Use this sheet to gain approval.
